Case Manager

Immediate opening (2ea) for civilian medical case manager. Both positions are funded through the end of the calendar year (31 Dec 11). Duty location is medical branch at Fort Ben-Harrison. Resumes must be received no later than close of business on 16 August 2011. Job description is as follows:


* Report to the Commander or his/her designated representative and serve as unit POC for all medical readiness issues.

* Assist commanders in reaching their goal of a full, medically ready Unit Manning Roster (UMR).

* S/he will segregate Soldiers who are not medically ready from those who are medically ready and direct their documentation to the proper personnel in Indiana Medical Branch which best suits their current situation. (Note: this includes forwarding weekly potentially not ready reports to medical branch).

* Send code requests to MND Section lead in order to code SM’s 999e or 999M as needed.

* Serves as liaison between medical branch and Med NCOs.

* Responsible for creating and updating referrals in eProfile.

* Responsible for contact with Soldiers in distress at Level 2 and Level 3 of Suicide Intervention.

* Required to make contact with Soldiers twice weekly and document conversations in eProfile for Soldiers with behavioral health issues.

* Provides case management support when tasked for pre-SRP/PHA/PDHRA events, which does include weekly PHAs at the PEC.

* Ensure the 2766 is complete and provide complete documentation to support the fact that a Soldier is deployable.

* Collect medical documentation from Med NCOs, review and submit to providers, in order to clear Soldiers, profile Soldiers, request boards or appointments as needed, etc.

* If trained, perform duties as Victim Advocate for Sexual Assault Response Team, as needed.

* Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines.
