General Duty Description: Soldier's will be involved in the data clean up for the integration of GIMS.

NATIONAL GUARD MEMERBSHIP IS REQUIRED. Please review the following information about this position and to submit your application:

SOLDIER WILL PROVIDE:

* J1 ADOS checklist and DA 1058-R (Application for ADOS Tour),
fill
out part 1, Soldier will sign blocks 22 & 24. Commander will sign block 35e.

* Provide DA Form 705 (PT Card) with passing score within 6
months
* DA Form 5500/5501-R if not in height/weight standard within 6
months
* Must be current on PHA (Green in AKO)
* If applicable DA Form 3349 physical profile (permanent or
temporary)
* For females, proof of a negative pregnancy test within 15 days
Of start of an Active Duty tour in excess of 30 days is required by AR 40-501, para 10-4.
* For females, proof of current Pap Smear.
* The rest of the documents on the checklist can be obtained
when the packet is turned in to records custodian.
